Insurance & Payment
Insurance:
We accept Aetna, BCBS, Cigna, Medcost, Private Pay, TRICARE, and UnitedHealthcare as payment options. It's essential to note that although we accept these providers, we cannot guarantee that your services will be fully covered. If you have any inquiries about your coverage, we recommend contacting your insurance company for clarification.
We do not accept BCBS ACA Plans, Blue Home, Blue Value, or Blue Local insurance plans.
Payment:
We welcome various payment methods, including all major credit cards, cash, Cash App, FSA (Flexible Spending Account), and HSA (Health Savings Account).

Cancellation & Inclement Weather Policy:
At 180 Evolution Therapy, we value your time and that of our providers. We kindly ask for a 24-hour notice if you need to cancel or reschedule your appointment. Please log in to the client portal to do so. If you don't see the cancellation option within the portal, the 24-hour notice period has passed, and we will charge the full session fee to your card on file for the missed appointment. Failing to provide notice is considered a "NO SHOW" and may affect your ability to schedule future appointments.
If severe weather is expected, clients scheduled for in-person sessions will receive an email notification 60 minutes prior to their appointment, informing them if their session will be conducted virtually instead.
To ensure fairness and consistency for all clients, the following cancellation policy applies:
Cancellations must be made through the client portal. We do not accept cancellations by phone, text, or email. This ensures accurate record-keeping, timely confirmations, and better access to open appointments for other clients.
24-Hour Notice Required: Clients must cancel or reschedule at least 24 hours before their scheduled appointment.
Late Cancellations & Missed Appointments: If less than 24 hours’ notice is given, or if you do not attend your session, a cancellation fee will be charged. This fee cannot be billed to insurance and is the client’s responsibility.
Good Faith Estimate “No Surprises Act”:
Under the law, healthcare organizations must give patients who don’t have insurance or are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs, such as medical tests, prescription drugs, equipment, and hospital fees.
Make sure your healthcare organization gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare organization and any other organization you choose for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
